Saima was introduced to the film industry by Nagina Khanum and made her debut in the 1987 film Greban. She then starred in her second film Khatarnaak, directed by Akram Khan. In her early years in the film industry, she was most often paired with actor Sultan Rahi in Punjabi films, but was recognized as a leading lady when director Syed Noor launched her career in Urdu films. Her biggest commercial success came in 1998 when she starred in the romance musical Choorian which grossed around Rs. 200 million and became Pakistan's highest-grossing Punjabi language film, making her the leading lady of Lollywood. She played the lead role of Tanya in the acclaimed revenge thriller Khilona, starring Meera and Saud. In 2000, she played a brave girl in the film Queen of the Jungle, which is a female Tarzan type who lives in the forest, swings on vines, rides elephants, etc. Directed by her husband Syed Noor. In 2005, he appeared as a snake in the supernatural fantasy film Naag aur Nagin. In 2011 she played the role of Munniya in the action film Bhai Log which was a moderate box office success and earned over Rs. 9.7 million in the first three days of its release.
In 2012, she starred alongside Shaan in the family film Shareeka which had a great start at the box office, earning over Rs. 3 million in the first three days of play alone. Saima also appeared in the biographical drama Salute based on the life of Senator Aitzaz Hassan.
In 2022, he worked in the Punjabi film Tere Bajre Di Rakhi.
Apart from films, he has also acted in a number of television series including Rang Laaga, Kaneez, Ye Mera Deewanapan Hai and Mubarak Ho Beti Hui Hai. In 2018, he appeared in the drama series Lamhay with Sarmad Husat. Although she has a romantic relationship with Syed Noor, it has not been officially announced whether she has married him or not. In 2007, Saima publicly stated during a press conference that she had married Syed Noor in July 2005 while filming their joint film Majajan.
In 2018, several media publications and online websites reported that Syed Noor was divorcing Saima and the two were living separately. However, the couple denied these rumors and posted a short video on social media, in which they stated that they are happily married and will never part.
She will appear alongside Moammar Rana in her husband Syed Noor's much-delayed film Bhai Wanted. Saima rose to prominence for her role in the film Choorian (1998), which is considered one of the highest-grossing Pakistani films of all time. Some of his other notable film roles are Buddha Gujjar (2002), Majajan (2006) and Bhai Log (2011), all of which were commercially successful. She was one of the country's leading film actresses in the 1990s and early 2000s.
Saima's cinematic work spans many genres, including the supernatural film Naag aur Nagin (2005) and the biographical film Salute (2016). She has also built a career in the Pakistani television industry, starring in various television series including Rang Laaga (2015), Yeh Mera Deewanapan Hai (2015) and Babban Khala Ki Betiyann (2018–2019).
In 2005 she married director Syed Noor, with whom she worked on several films.Saima was one of the most popular and prominent film actresses in Pakistan in the 1990s and early 2000s. In 2017, The Express Tribune ran an article about the lack of new heroines in the Lollywood revival, citing Saima as a fortune-teller for the industry because she hails from South Punjab. Film critic Omair Alavi of The News International praised his acting credibility, writing, "You can see why the director has kept casting him over the years." In 2010, BBC News called her "the reigning queen of Pakistani screens" and noted that she was "now one of the biggest names in the business".
After the decline of the Pakistani film industry, Saima made her television debut and became one of the highest-paid actresses in television media along with actress Resham, her contemporaries from the 1990s.
